Job description

Core Schedule: Monday-Friday, 9am-5:30pm, with potential changes based on client needs.

Role Summary

Provides planning, implementation, monitoring, and evaluation to meet the treatment needs of young adults. Manages the Extended Reach system related to client filing and maintains the e-file throughout the client’s stay in the supervised independent living program.

Essential Functions
  • Coordinate and connect care for all clients within the Supervised Independent Living Program.
  • Assess young adult needs, ensure connections to treatment team and outside resources, and follow up accordingly.
  • Serve as the initial contact for referral sources including DCBS workers and community partners.
  • Facilitate weekly treatment team meetings and coordinate schedules.
  • Communicate treatment concerns, safety issues, and client needs with team members, following BGH policies.
  • Open and maintain comprehensive case files, ensuring clients understand program requirements, goals, and rights.
  • Update and maintain contact information for DCBS and clients.
  • Participate in new client admissions with program leadership.
  • Ensure completion of necessary paperwork, document client progress, and compile reports.
  • Manage monthly and weekly connections to outside agencies such as DCBS and Aetna.
  • Complete required state documentation, including 1294 A’s and B’s.
  • Collaborate on discharge planning and connect outside supports for successful transition.
  • Participate in URC bi-weekly meetings and collaborative efforts within BGH programs.
  • Assist with transitions between programs through proper documentation and communication.
  • Ensure discharge documentation accurately reflects client needs.
  • Send discharge notifications to supervisors or designees.
  • Meet regularly with supervisors for professional development and administrative review.
  • Coordinate referrals for CSA and OT services, ensuring proper documentation for billing.
  • Participate in on-call rotations.
Unplanned Activities

Assist with additional activities as needed to meet organizational goals, supporting teamwork and flexibility.

Qualifications
  1. Education: Bachelor's degree in social services or related field; Master’s preferred.
  2. Experience/Credentials: At least two years working with adolescents and young adults in related settings; proficiency with relevant software.
  3. Other: Professional interaction, confidentiality, legal work eligibility, valid driver’s license, background checks, and health screenings.
Physical Demands

Ability to perform physical activities including lifting up to 40 pounds, and visual acuity for various tasks.

Work Environment

Exposure to client environments, moderate noise levels, and contact with community members and staff.
