Job description

Schedule: Monday-Friday, 9am-5:30pm, subject to change based on client needs.

Role Summary

Provides planning, implementation, monitoring, and evaluation of actions to meet young adults' treatment needs. Manages the Extended Reach system related to client filing and cares for the e-file throughout the client’s stay in the supervised independent living program.

Essential Functions
  • Coordinate and connect care for all clients within the Supervised Independent Living Program.
  • Assess young adults’ needs, ensure connections to treatment team and outside resources within 3 days of admission.
  • Serve as initial contact for referral sources, including DCBS and community partners.
  • Facilitate weekly treatment team meetings, coordinate schedules, and invite team members.
  • Communicate treatment concerns, safety issues, and needs with team members, following BGH policies.
  • Open and maintain case files, ensure clients understand program requirements, goals, and rights.
  • Update and maintain contact information for DCBS and clients.
  • Participate in new client admissions with Program Supervisor and leadership.
  • Ensure completion of necessary paperwork, document client progress, and compile reports.
  • Maintain monthly and weekly connections with DCBS, Aetna, and other supports.
  • Complete monthly documentation, including 1294 A’s and B’s.
  • Collaborate on discharge plans and connect outside supports for successful transition.
  • Participate in URC bi-weekly meetings and collaborative efforts.
  • Assist with transitions between programs and document at discharge.
  • Send discharge notifications to supervisors.
  • Meet regularly with supervisors for professional development and administrative issues.
  • Coordinate referrals for CSA and OT services, ensuring billing documentation is accurate and timely.
  • Participate in on-call rotations.
Unplanned Activities

Assist with additional activities as needed to meet organizational goals, including other duties as assigned.

Qualifications
  1. Education: Bachelor’s degree in social services or related field; Master’s preferred.
  2. Experience/Credentials: Minimum two years working with adolescents and young adults in relevant settings. Proficiency with Windows, Microsoft Office, and related software.
  3. Other: Professional interaction, confidentiality, age at least 21, valid driver’s license, background check, and TB screening required.
Physical Demands

Ability to stand, walk, sit, manipulate objects, reach, climb, balance, stoop, kneel, crouch, talk, hear, taste, smell, and lift up to 40 pounds occasionally. Vision abilities include close, distance, color, peripheral, depth perception, and focus adjustments.

Work Environment

Exposure to client apartments, communal areas, and office settings. Variable cleanliness and moderate noise levels. Interaction with clients, staff, and community members.
