A leading organization is seeking a professional to manage care in its Supervised Independent Living Program for young adults. The role requires a Bachelor's in social services and two years of experience, focusing on coordination and documentation to ensure effective client treatment and support. The position involves physical demands typical of social service roles and offers competitive benefits including health insurance and a 401k match.
Schedule: Monday-Friday, 9am-5:30pm, subject to change based on client needs.
Provides planning, implementation, monitoring, and evaluation of actions to meet young adults' treatment needs. Manages the Extended Reach system related to client filing and cares for the e-file throughout the client’s stay in the supervised independent living program.
Assist with additional activities as needed to meet organizational goals, including other duties as assigned.
Ability to stand, walk, sit, manipulate objects, reach, climb, balance, stoop, kneel, crouch, talk, hear, taste, smell, and lift up to 40 pounds occasionally. Vision abilities include close, distance, color, peripheral, depth perception, and focus adjustments.
Exposure to client apartments, communal areas, and office settings. Variable cleanliness and moderate noise levels. Interaction with clients, staff, and community members.
