Job summary

National Youth Advocate Program seeks a part-time Treatment Coordinator to support youth and families in Macon, GA. Under supervision, you will coordinate services, provide advocacy, and help families access mental health, medical, educational, and social supports.

The role requires a Bachelor’s degree in social work or related field and experience with youth and families. You must have a valid driver’s license, willing to work non-traditional hours, and comfort serving diverse communities.

Qualifications

• Bachelor’s degree in social work or comparable human services field from an accredited institution.
• 2 years of work experience in direct service with youth and families preferred.
• Valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
• Willingness to work flexible and non-traditional hours in service of foster caregivers, families of origin, and persons served.
• Ability to be flexible and respectful to persons from diverse cultures, backgrounds, and beliefs.
• Proficient use of computers and common software (word processing, spreadsheets, databases).

Responsibilities

• Provides case management, transportation, and professional services meeting outcomes for persons served.
• Travels daily to provide community-based services in compliance with requirements.
• Completes required documentation and training in a timely manner.
• Coordinate and monitor youth and family services including mental health, medical, education, and social services.
• Review and modify youth-based plans to ensure progress toward goals.
• Assist in family reunification or stabilization with youth’s primary family or permanent living arrangement.
• Provide safety assessment, crisis response, and behavioral stabilization as needed.
• Participate in Continuous Quality Improvement activities monthly/quarterly.
