Job summary

Harbor Light Mission in Michigan seeks a dedicated Case Manager to support treatment clients. You will conduct didactics, manage caseloads, maintain records, and collaborate with clinical staff to ensure quality care.

Requires a BA in social work or related field, MCBAP credential or plan within 2 weeks, TB test and police clearance, and ability to work with multi-problem individuals. Experience with mental health or substance abuse is preferred.

Qualifications

• Bachelor's degree in social work or a comparable human services field is required.
• Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date.

Responsibilities

• Facilitate didactic groups and case management services.
• Conducts didactics/educational lectures.
• Maintain case records in accordance with Harbor Light standards.
• Serve as Case Manager to treatment clients.
• Assists clinical staff with various tasks as assigned (packets, nonclinical support).
• Participates in review of clinical issues, activities and policies.
• Attends clinical staff meetings and team conferences.
• Participates in ongoing educational activities to enhance treatment skills.
