Job summary

WelbeHealth is looking for a Traveling Physical Therapist to coordinate and provide restorative therapy to participants at day centers and in their homes. You will work closely with the PACE physician and IDT to maximize independence and safety, and to restore function.

Requirements include a Bachelor’s degree in physical therapy, CA license or eligibility, and five years of experience with elderly populations. This role involves travel and direct patient care in diverse settings.

Qualifications

• Bachelor’s degree in physical therapy is required; master’s degree preferred.
• California Physical Therapy licensure or eligibility.
• Minimum five years of progressive physical therapy experience.
• Two years in a clinical setting with frail or elderly populations.
• Ability to work independently with minimal supervision.

Responsibilities

• Conduct physical therapy assessments in facilities and participant homes to define care needs, preferences, and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document physical therapy interventions per physician orders and care plans.
• Provide active and passive exercise, gait training, ADLs, and related modalities.
• Oversee Therapy Assistants, providing training to implement Functional Maintenance Programs.
• Train staff, caregivers, participants, and families in use and care of therapeutic equipment.

Job description

At WelbeHealth, each participant of our program is guided by our interdisciplinary team (IDT) composed of clinical and non-clinical members who work cohesively to all-inclusive care to the most vulnerable senior population. Our core values and participant focus lead the way no matter what.

The WelbeHealth Traveling Physical Therapist is accountable for coordinating and providing restorative and rehabilitative physical therapy services to participants at multiple day centers and at participants’ homes as assigned. This position works in close coordination with the PACE Physician, rehabilitation staff, and other IDT members to maximize participant independence and safety, and to develop and restore function.

Essential Job Duties

• Conduct physical therapy assessments in our facilities and in participant homes to define care needs, preferences, and goals of participants
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document physical therapy interventions in accordance with physician orders and/or as agreed upon in participants’ care plans
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, as well as treatments such as massage, heat, sound, water, light, or electricity
• Oversee the Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, caregivers, participants, and families in the use and care of therapeutic equipment

Job Requirements

• Minimum of five (5) years of progressive physical therapy experience
• Minimum of two (2) years of physical therapy experience in a clinical setting with a frail or elderly population
• Bachelor’s degree in physical therapy required; masters’ degree preferred
• Ability to work independently with minimal supervision
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
