Job summary

Beacon Specialized is seeking a Licensed Practical Nurse to partner with the Director of Nursing and oversee care for individuals served. The role travels to homes and BSLS sites, delivering high-quality care, medication management, and education with a collaborative team.

You will participate in interdisciplinary meetings, support Direct Service Providers, and ensure compliance with policy and regulatory requirements while maintaining excellent English communication and computer skills.

Qualifications

• LPN license required in state of licensure.
• Minimum 2 years of experience preferred.
• Background check and valid driver’s license required.
• Proficient in English (reading/writing).
• Ability to use office equipment and BSLS IT systems.

Responsibilities

• Provide medication management and vaccinations as needed.
• Complete nursing assessments and diabetic foot assessments.
• Coordinate care across medical, psychiatric, dental, and optical services.
• Travel to BSLS locations and homes to provide care.
• Participate in interdisciplinary team meetings and coordinate with Direct Service Providers.
• Monitor health concerns and follow up on changes or orders.
• Ensure compliance with company policies and regulatory requirements.
