Job summary

Curana Health is seeking a Traveling HRA Nurse Practitioner to perform comprehensive, non-invasive physical assessments in patients' residences across Long Term Care, Assisted Living, and Independent Living settings. This full-time role involves regional travel and completing 20–30 assessments per week with careful documentation and patient education.

The NP will collaborate with Curana and facility teams to ensure continuity of care, while maintaining regulatory compliance and confidentiality.

Qualifications

• Active Nurse Practitioner licensure in two or more states.
• Unrestricted licensure in markets with ability to obtain more licenses within 30 days.
• Extensive experience with ICD-10 and HCC documentation.
• Proficiency with EMR systems and patient data entry.

Responsibilities

• Travel to patients' residences within assigned regions.
• Perform comprehensive health risk assessments and physical examinations.
• Document assessments accurately and timely.
• Conduct medication reconciliation to ensure safety.
• Provide basic education on wellness and preventive care.
