TALENT Software Services in Midland, MI is seeking a Physical Therapist to provide medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and Standards of Practice.

You will assess patients, implement plans, educate teams and families, and participate in QAPI; supervise PTAs and aides; travel up to 60 miles; Monday–Friday 8a-5p; HCHB required.

Qualifikationen

• Knowledge of physical therapy principles for terminally ill patients.
• Demonstrated problem-solving and decision-making abilities.
• Ability to manage patient care with bedside nursing, symptom management, crisis intervention, and family intervention.
• Excellent communication skills.
• Ability to develop and maintain rapport with patients, families, and staff.

Aufgaben

• Participate in developing patient plans of care.
• Conduct patient assessments and communicate with physicians before start of care.
• Implement the plan of care and document care provided in medical records.
• Provide education to team members, patients and families.
• Participate in QAPI programs as needed.
• Support growth and profitability through proper use of resources and community education.
• Attend interdisciplinary team meetings to coordinate care.
• Apply agency policies and regulatory rules in care provision.
• Provide skilled restorative treatments to reduce pain and restore function.
• Conduct initial and ongoing patient assessments including OASIS at appropriate times.
• Evaluate outcomes of care and adjust as needed.
• Direct and supervise PTAs, CNAs, and HHAs with supervisory visits.
