Job summary

TALENT Software Services is seeking a Physical Therapist to provide medically prescribed, goal-oriented therapy in a home health setting across Michigan. You will follow physician plans of care, document care, and coordinate with physicians to optimize patient outcomes.

This role requires strong communication, multidisciplinary collaboration, and the ability to educate patients and families while contributing to quality improvement initiatives.

Qualifications

• Knowledge of physical therapy principles and standards of practice.
• Strong problem-solving and decision-making abilities.
• Ability to manage patient care with bedside nursing and family involvement.
• Excellent communication skills with patients, families, and staff.
• Ability to work effectively in multidisciplinary teams.

Responsibilities

• Develop patient physical therapy plans of care.
• Perform visits for assessments and communicate with physicians as needed.
• Implement the plan of care and document accurately in patient records.
• Provide education and instruction to patients, families, and team members.
• Participate in quality assessment and performance improvement programs.
• Contribute to growth through responsible resource use and community education.
• Coordinate care in interdisciplinary meetings and support staff.
• Apply agency policies and ensure compliance with regulations.
• Provide skilled restorative treatments and conduct safety instructions.
• Conduct initial and ongoing assessments including OASIS.
• Evaluate outcomes of care.
• Direct and supervise PTAs, CNAs, and HHAs.
• Accept direction from leadership.
