Qualifications

• Must hold a valid Speech Language Pathologist license in the state of practice.
• ASHA CCC or Clinical Fellowship eligibility required.
• Valid driver's license or reliable transportation to patients' homes.
• Experience in in-home or outpatient rehabilitation preferred.

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings.
• Assess and treat speech, language, communication, and swallowing disorders.
• Collaborate with the rehabilitation team and physicians to plan care.
• Document clinical findings and treatment progress in a timely manner.
• Supervise CFYs, students, and support personnel as permitted by licensure.
• Adapt plans of care to patient needs and safety requirements.
• Coordinate care across home and community environments.
