Job summary

Proforma Health Inc. seeks a Travel Speech-Language Pathologist (SLP) to support Long-term Acute Care settings.

You will assess, diagnose, and treat residents with communication, cognition, and swallowing disorders, collaborating with a multidisciplinary team to develop treatment plans and document progress. You will ensure regulatory compliance, maintain documentation, and provide high-quality care across shifts.

Qualifications

• 1+ year of SLP experience, preferably in LTC or rehab settings.
• MO State License and CCC certification required.
• New graduates with Level II experience and 3+ years of CCS experience are welcomed.
• BLS certification, SSN, DOB, and recent references required.

Responsibilities

• Assess, diagnose, and treat residents with communication, cognition, and swallowing disorders.
• Collaborate with a multidisciplinary team to develop individualized treatment plans, document progress, and ensure quality care.
• Provide therapy services, collaborate with healthcare teams, and maintain documentation.
• Ensure compliance with licensure and certification standards.
