Job summary

Cross Country Allied is seeking a travel Physical Therapist for a Skilled Nursing Facility in Ada, Oklahoma. The PT will screen, evaluate, and treat patients with physical functioning disorders and wound care needs, developing and implementing plans while adhering to regulatory standards.

This travel role requires a valid Oklahoma PT license, 1 year of PT experience including LTC or rehab, and the ability to obtain necessary equipment. Schedule is days, 40 hours per week, 8 hours per day.

Qualifications

• Valid Oklahoma physical therapy license required.
• Minimum 1 year PT experience including long-term care or rehab.
• Degree in Physical Therapy from an accredited program; ability to obtain DME/supplies as needed.

Responsibilities

• Screen, evaluate, and rehabilitate patients in SNF setting.
• Develop and implement appropriate treatment plans.
• Monitor progress and adjust treatments; document activities in EMR.
