Qualifications

Responsibilities

• Screen, evaluate, and treat communication, cognitive, and swallowing disorders.
• Develop and adjust individualized plans of care per standards.
• Use standardized assessments and evidence-based practice.
• Document treatment and progress in EHR system.
• Provide patient, family, and caregiver counseling and education.
• Collaborate with supervisors and interdisciplinary team on progress.
• Complete discharge summaries per regulatory requirements.
• Record treatment minutes per standards.
