Qualifications

• BLS and ACLS certifications required.
• Active RN license in the state.
• Minimum two years bedside experience; ICU LTACH experience preferred.

Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission.
• Develop individualized care plans and participate in rehabilitation activities.
• Initiate discharge planning from the day of admission and educate patients on post‑discharge care.
• Document assessments, interventions and patient responses accurately per hospital policy.
