Job summary

IDR Healthcare is seeking a travel RN – Home Health for Charlottesville, VA. The 13-week assignment involves 40 hours/week, with 8-hour day shifts and a flexible schedule.

Expect comprehensive home visits, interdisciplinary collaboration, and HIPAA-compliant documentation to ensure optimal patient outcomes. The ideal candidate will have an Associate’s Degree in Nursing, a current compact RN license, 1–2 years of home health experience, and proficiency with OASIS and HCHB systems.

Qualifications

• Associate’s Degree in Nursing; BSN preferred.
• Current, unrestricted multi-state RN license (Compact).
• 1–2 years of clinical home health experience preferred.
• Current CPR/BLS certification.
• TB test clearance.
• Wound care/competency test completion.
• Case Management Certification preferred.
• Proficiency with OASIS and Homecare Homebase (HCHB).

Responsibilities

• Conduct comprehensive assessments for high-risk members and patients in their home setting.
• Collaborate with primary care providers and other healthcare professionals to develop and implement individualized, goal-oriented care plans.
• Identify and coordinate cost-effective, appropriate levels of care across the continuum.
• Facilitate non-medical supports to promote adherence to care plans.
• Manage and coordinate care transitions to ensure seamless continuity between providers, facilities, and home settings.
• Partner with physicians, social workers, discharge planners, and claims professionals to support optimal patient outcomes.
• Maintain detailed documentation of clinical, functional, and financial outcomes.
• Identify opportunities for health promotion, illness prevention, and patient education.
• Act as a patient advocate while maintaining strict confidentiality and HIPAA compliance.
• Support the prevention and early intervention of adverse patient outcomes.
