Job summary

IDR Healthcare is seeking a travel nurse RN Home Health for a travel nursing job in Harker Heights, Texas. The assignment is 13 weeks long, with 40 hours per week and 8-hour daytime shifts.

You will provide personalized home health care, perform assessments, and coordinate with the interdisciplinary team to deliver high-quality care in patients' homes. Current RN license in the state, minimum 1 year clinical nursing experience, and willingness to travel up to 50% are required; ASN/BSN preferred,

Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience in a clinical care setting.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Willingness and ability to travel within assigned service area (up to 50%).
• Associate’s or Bachelor’s Degree in Nursing (preferred).
• Prior experience in Home Health or Hospice (preferred).
• Familiarity with HomeCare HomeBase or similar EMR systems (preferred).

Responsibilities

• Perform initial and ongoing assessments of patients’ physical, psychological, social, and environmental needs.
• Develop and implement individualized care plans in coordination with patients, families, and the interdisciplinary team.
• Reassess patients regularly and adjust care plans as needed based on changes in condition or circumstances.
• Communicate significant findings and changes in patient status to supervisors, physicians, and other care team members.
• Maintain accurate clinical documentation in compliance with regulatory and organizational standards.
• Uphold patient confidentiality and privacy in accordance with HIPAA and other regulations.
• Collaborate with other clinicians to ensure coordinated, efficient, and high-quality care.
