Responsibilities
- Provides, coordinates, and directs the provision of home health nursing services and case management processes according to the patient's individualized plan of care, through the competent application of nursing process. Requires extensive travel throughout the assigned region to provide patient care.
- Demonstrates knowledge of home care and competency in discipline-specific patient care skills, required by the care center for patient care.
- Performs comprehensive assessments and collaborates with the multidisciplinary care team and other healthcare professionals to develop, modify, and implement an individualized plan of care that ensures quality, proper discharge planning, and achieves desired outcomes.
- Implements appropriate nursing programs and initiatives to achieve desired outcomes.
- Provides clinical episode management, including oversight of the patient's plan of care and necessary care plan changes.
- Utilizes care center resources and standards of practice to deliver high-quality care and achieve patient outcomes.
- Makes referrals to other disciplines as needed and documents rationale.
- Promotes patient health and independence through teaching and rehabilitative measures, and advocates for patient participation in care planning.
- Supervises caseload including LPNs and Home Health Aides, and facilitates care coordination with PRN staff.
- Completes documentation accurately and timely, including assessments, care plans, and physician orders.
- Ensures continuity of care through effective communication with team members and community resources.
- Participates in team conferences and leads discussions on multidisciplinary cases.
- Maintains communication with the clinical manager regarding patient progress.
- Participates in quality improvement initiatives and incorporates them into patient care planning.
- Provides staff education, competency validation, and utilizes teaching materials for clinicians and patients.
- Performs on-call duties as assigned.
- Monitors compliance with third-party payer requirements.
- Respects patient and co-worker confidentiality and privacy.
- Performs other duties as assigned.
Qualifications
- Current, unencumbered RN license for the state(s) of practice. May possess single-state or multi-state (Nurse License Compact) license.
- Current CPR certification.
- At least 2 years of clinical home health RN experience.
- Experience with Homecare Homebase EMR.
- Valid Driver's License and reliable transportation with agency liability insurance.
- Familiarity with computer use, including email, internet, and online HR/training systems.
