Qualifikationen

• Active AK RN license required and must be in hand prior to consideration.
• Current BLS certification.
• Minimum of 1 year of recent Home Health experience required.
• Proficiency in HCHB (Homecare Homebase) EMR required.
• OASIS experience required for those completing SOC documentation.
• Must maintain a minimum of 30 productivity points weekly.
• Active driver's license and personal transportation required.

Aufgaben

• Home Health setting serving a geriatric patient population.
• Average caseload of 5-7 patients per day.
• Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM).
• Territory covers up to a 60-mile radius from the hub office.
• No on-call requirement, though an on-call rotation may be discussed during interview.
• Travel pairs accepted, with each traveler assigned to their own territory.
• Provide skilled home health nursing care to geriatric patients in their residences.
• Timely communication and documentation are mandatory.
• Monday orientation required, including completion of billable training modules.
• Hospice/Home Health checklist completion is mandatory.
• Dress code is scrubs of any color; jeans are not permitted.
• iPad provided for work use; personal cell phone use is not required.
• Mileage reimbursed at the IRS standard rate.
• Local candidates accepted at the travel rate.
• First-time travelers will not be considered for this position.
• Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler.
• Requested time off (RTO) must be submitted at the time of application.
