ConnectedHealth is seeking a Long-Term Care Registered Nurse for a 13-week travel assignment in Appleton, Minnesota. The role centers on delivering geriatric nursing care, collaborating with an interdisciplinary team, and maintaining safety and compliance standards.

You will provide direct patient care, monitor conditions, administer medications, and document care activities. A valid RN license, BLS, CPR, and at least 1 year in geriatric LTC are required; 12-hour day shifts are available.

Qualifications

• Valid RN license from an accredited nursing program.
• Current BLS certification.
• Current CPR certification.
• Minimum of 1 year experience in long-term care or geriatric nursing.
• Ability to work 12-hour day shifts.
• Strong communication skills.
• Compassionate bedside manner.

Responsibilities

• Provide high-quality nursing care to geriatric patients in a long-term care setting.
• Collaborate with interdisciplinary team members to develop and implement patient care plans.
• Monitor patient conditions, administer medications, and document all care activities accurately.
• Educate patients and their families on health management, discharge instructions, and available resources.
• Maintain a safe, clean, and organized environment to promote patient well-being and prevent infections.
