Job summary

American Traveler Staffing Professionals is hiring an RN Case Manager for a Home Health setting in Oregon. Active RN license required with prior acute care or home health case management experience. Day shifts, 40 hours weekly, 13-week assignment, Epic EMR used, CRASE/BSN preferred.

Qualifications

• BLS certification preferred.
• CRASE training required; may be needed for extended assignments.
• Minimum two years of experience required.
• Clinical experience in acute care or home health assessing needs and resources.

Responsibilities

• Assess patients' healthcare needs and develop comprehensive care plans.
• Coordinate transitions between care settings to ensure continuity of care.
• Evaluate medical necessity for treatments and obtain insurance authorizations.
• Monitor length of stay and discharge barriers.
• Educate patients and families on diagnoses, treatments, and self-care.
