Job summary

Your Health Organization in the Columbia area is seeking a Nurse Case Manager to support patients across facilities and homes. The role involves visiting patients, coordinating with providers, and initiating telemedicine visits as needed.

Travel is required daily. This is a full-time position with a 12-hour shift track system (7 days on, 7 days off) serving Irmo, Lexington, and Columbia communities.

Qualifications

• Must be a licensed nurse. RN preferred. License in good standing with the appropriate board.
• 3+ years of clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal communication skills.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records.
• Complete consents with patients.
• Enroll patients in Care Management, if eligible.
• Initiate a Care Management Plan of Care, if eligible.
• Coordinate with the patient’s health care team and providers.
• Travel to patient homes, long-term care facilities, etc., daily.
• Evaluate for home health, hospice, RPM devices, or related resources.
• Communicate and coordinate care across teams.
• Reconcile prescribed and OTC medications and supplements.
• On call may be required.
