Job summary

BrightStar Care Plymouth seeks a Full-Time Registered Nurse (RN) Case Manager to deliver in-home care and supervision across our territory. This traveling role offers high autonomy with a small initial caseload and a schedule you can set.

Responsibilities include supervising caregivers, completing assessments, coordinating with providers, documenting care in the eCharting system, and covering for others when needed. Requirements: MN RN license, CPR, ability to travel, and valid driver’s license.

Qualifications

• Active MN Board of Nursing license required.
• Current CPR certification required.
• Minimum of one year nursing experience preferred but not required.

Responsibilities

• Provide in-home care based on personalized plan of care.
• Oversee caregivers and perform supervisory visits and competency testing.
• Perform and maintain ongoing client case management and documentation in eCharting.
• Coordinate with other agencies, healthcare providers, pharmacies, and medical supply companies.
