Job summary

Vibra Travels LLC is seeking a Case Manager to join our expanding network of healthcare professionals. You will coordinate clinically complex patient care across a continuum and ensure quality outcomes while traveling to various facilities nationwide.

The role emphasizes discharge planning, resource coordination, and addressing issues to maintain service quality. Active RN or LSW licensure is required in the employment state, with travel ability and BLS certification as part of qualifications.

Qualifications

• One year of case management experience preferred.
• CCM or ACM certification preferred.
• Must hold active RN or LSW license in the employing state.
• Travel ability with air and ground transportation.

Responsibilities

• Coordinate clinically complex patient care across a continuum to achieve quality outcomes and appropriate services.
• Assist with discharge planning and resource coordination.
• Address issues and concerns at the facility to maintain service quality and cost effectiveness.
• Provide case management services to rehabilitation-focused patient populations.
