Qualifications

• Eighteen months of recent experience in an Acute Care Case Manager setting.
• Active RN License.
• BLS Certifications.
• Degree from accredited nursing program.

Responsibilities

• Coordinate continuity of care for patients and act as liaison between patient families and healthcare organizations.
• Promote self-managed care and efficient use of healthcare resources.
• Ensure proper treatment is administered at the right time to maximize health outcomes.
