Assignment length is 13 weeks, with 8-hour shifts totaling 40 hours per week, start ASAP. You will document care, administer therapies, and coordinate across providers to ensure continuity of care. Weekly pay and comprehensive benefits are included.

Qualifications

• 2+ years of nursing experience.
• Experience in case management or care coordination.

Responsibilities

• Document care delivered, patient response, and communications in the medical record.
• Administer ordered therapies and monitor for adverse reactions or changes in condition.
• Work 40 hours per week with 8-hour shifts.
• Perform medication reconciliation activities as directed and communicate discrepancies.
• Support care coordination activities across providers to promote continuity of care.
