Qualifications

• At least one year of Case Management experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Coordinate continuum of care activities for assigned patients.
• Ensure optimum utilization of resources, service delivery, and compliance with medical regimes.
