Qualifications

Responsibilities

• Coordinates management of care and ensures optimum utilization of resources, service delivery and compliance with external review agencies.
• Provides ongoing support through assessment, care planning, plan implementation and evaluation of patient needs.
• Performs discharge planning and care coordination to promote continuity of care and cost effectiveness.
• Advocates for patients through resource utilization and holistic needs assessment.
