Qualifications

• Eighteen months of recent experience in an Acute Care Case Manager setting.
• Active RN License.
• BLS Certifications.
• Degree from accredited nursing program.

Responsibilities

• Coordinate continuity of care for patients and act as liaison between family and healthcare organization.
• Promote self-managed care and optimal use of healthcare resources in a cost-effective way.
• Ensure timely and appropriate treatment to maximize health outcomes.
