Job summary

Talented Medical Solutions is seeking an experienced Registered Nurse (RN) Case Manager to join a collaborative care management team in Lodi, CA. This assignment runs May 11, 2026 – August 9, 2026, with Day Shift hours: 8:00 AM–6:30 PM, 4x10s (40 hours/week) and occasional weekends.

You will coordinate care across the continuum from pre-admission through post-discharge, guide discharge processes, and act as a liaison among patients, families, physicians, and payors to ensure optimal outcomes and

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of acute care case management experience.
• BSN and CCM are preferred.

Responsibilities

• Coordinate and oversee patient care plans across disciplines, ensuring timely and appropriate care.
• Collaborate with physicians on treatment plans, length of stay, and discharge planning.
• Monitor diagnostics, treatments, and procedures to align with clinical guidelines.
• Serve as liaison between patients, families, care teams, and payors.
• Focus on complex cases, including frequent ED utilizers and vulnerable populations.
• Facilitate transfers to tertiary care centers and ensure continuity of care.
• Support regulatory and accreditation standards.
• Analyze trends in frequent admissions to improve care strategies.
