Job summary

Talented Medical Solutions is seeking an experienced Registered Nurse (RN) Case Manager to join a collaborative care management team in Lodi, CA. This assignment runs May 11, 2026 – August 9, 2026, with Day Shift hours: 8:00 AM–6:30 PM, 4x10s (40 hours/week) and occasional weekends.

Position Overview

Monitor diagnostics, treatments, and procedures to align with clinical and coverage guidelines

Focus on complex cases, including frequent ED utilizers, chronic conditions, substance use, and vulnerable populations

Facilitate transfers to tertiary care centers and ensure continuity of care

Support compliance with regulatory and accreditation standards

Analyze trends in frequent admissions and contribute to care improvement strategies

Requirements

Mandatory:

Active RN license (state of practice)

Minimum 1 year of acute care case management experience

Preferred:

Bachelor of Science in Nursing (BSN)

Case Management Certification (CCM or equivalent)

Prior experience in a care management role
