Job summary

Focus Staff is seeking a traveling Registered Nurse (RN) Case Manager for a 13-week assignment in Vacaville, CA. This contract role supports a client as a travel clinician and offers the opportunity to work across shifts.

The RN Case Manager will perform case management services, coordinate care, and ensure patient safety while maintaining documentation and communicating with the care team.

Qualifications

• Current state license required.
• BSN or related health field preferred but not required.
• Minimum of three years hospital-based nursing with utilization/case management experience.
• BLS for Healthcare Providers within 30 days of hire.
• At least 2 years of work experience.

Responsibilities

• Provides case management services to patients according to policy and procedure.
• Take direction from others and follow through to promote efficiency.
• Communicate effectively with patients, families, and staff.
• Set priorities for care and coordination.
• Protect and advance patient’s best interests.
• Maintain knowledge of billing, payor contracts, and reimbursement issues.
• Demonstrate ongoing competency in job duties.
