Summary:

The Case Manager RN coordinates and facilitates patient care across the healthcare continuum, ensuring optimal use of resources and improved patient outcomes. Responsibilities include assessing patient needs, developing care plans, collaborating with multidisciplinary teams, and ensuring compliance with standards.

Essential Work Functions:

• Assess patients' medical, psychosocial, and functional needs
• Develop and update individualized care plans
• Coordinate transitions of care
• Facilitate communication among patients, families, and teams
• Monitor patient outcomes and adjust care plans accordingly
• Ensure regulatory compliance
• Advocate for patients' preferences
• Conduct utilization reviews
• Educate patients and families
• Document comprehensively in medical records
• Participate in team meetings
• Perform other duties as assigned
