Facility Details

• Type: Skilled Nursing Facility (SNF) and Long-Term Care (LTC)
• Bed Count: 60 SNF / 35–50 LTC
• Census: Typically 80–95 residents
• Room Type: 20 rooms are non-private
  
  

Staffing & Ratios

• RNs:
• LTC: 1:25
• SNF: 1:13
• LPNs:
• LTC: 1:25
• SNF: 1:13
• CNAs: 1:10–15 (goal is ~1:10)
  

Job Duties

• Administer IVs (antibiotics/fluids – no titratable drips)
• Assessments, medication administration
• Dressing changes, wound vacs
• Admissions/discharges (completed by RN)
• CNAs perform vitals and ADLs (no blood sugars)
  
  

Common Diagnoses

• Feeding tubes, IV antibiotics, post-surgical patients
• Rehab and long-term care residents
  
  

Team & Support

• RT On-Site: Day shift M–F; available for AVAP, CPAP, BiPAP
• Physician Coverage:
• NP on-site M–F
• 24/7 on-call hospitalist coverage
• Some patients followed by private MDs
• Interdisciplinary Team Includes:
• PT, OT, SLP
• Respiratory Therapy
• Social Workers, Dietitian
• MDS Nurses, Wound Care Nurse
  

Additional Facility Information

• Pharmacy: On-site, 24/7 access
• Omnicell and med carts in use
• Meetings: RNs encouraged to attend grand rounds and team meetings
• COVID Unit: Patients intermixed with general population
• PPE: Both PAPR and N95 masks available
  

Scheduling & Shifts

• RN/LPN Shifts: 5:00–5:00
• CNA Shifts: 8 or 12 hours
• Weekend Requirement: Every other
• Holiday Requirement: Every other
• Call/Standby: None
• Floating: May float between units; efforts made to maintain unit consistency
• Block Scheduling: Considered when possible
• Scrub Colors: Any professional scrubs allowed
  
  

Orientation

• One 12-hour floor shift
• Additional orientation available upon request if needed
