Job summary

Cornerstone Rehab, based in Kentucky, is seeking a full-time Physical Therapy Assistant to deliver patient-centered care to adult and geriatric patients. You will collaborate with a diverse clinical team and travel to nearby centers as needed.

The role involves documenting progress, discharging summaries, and developing plans of care aligned with patient needs. A PTA license and SNF/acute care experience are required for this position.

Qualifications

• Valid PTA License and Credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
