A valid State PTA license and credentials are required, with SNF or acute care experience preferred.

- Provide physical therapy to adult and geriatric patients
- Prepare documentation, progress notes, and discharge summaries
- Establish a plan of care in accordance with the patients' needs and capabilities
- Travel to nearby centers as needed
