Qualifications

• Active Iowa PT license or eligibility required.
• Minimum 1 year of professional physical therapy experience.
• Hospital or acute-care rehab experience preferred.
• Epic documentation experience preferred.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide interventions to improve mobility, strength, balance and independence.
• Monitor progress and adjust treatment plans based on response.
• Educate patients and caregivers on exercises and home programs.
• Collaborate with physicians, nurses, and rehab teams.
• Document evaluations, treatments, progress, and discharge planning in Epic.
• Support patient-centered care throughout rehabilitation.
