The role requires active CT licensure, outpatient experience, and strong documentation skills. Candidates must have recent unit or setting experience and be willing to travel within the designated radius.

Qualifications

• Active and unrestricted Physical Therapist license in Connecticut
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
• Adherence to professional code of conduct and ethical standards
• Completion of all state-mandated continuing education requirements

Responsibilities

• Complete comprehensive evaluations and develop/modify treatment plans
• Document medical records with goals, assessments, and necessity letters
• Design functional treatment programs to support community reintegration
• Delegate and direct therapy services for assigned patients
• Supervise students and provide evaluations
• Maintain licensure in good standing and comply with State Practice Act
