Ardor Health Solutions is seeking a Travel Physical Therapist for a Home Health assignment in Canonsburg, PA, near Pittsburgh. This role offers 40 hours per week with 12-week duration, serving patients in their homes and using OASIS and HCHB for documentation.

The ideal candidate holds an active PA PT license, has Home Health and OASIS/HCHB experience, and can travel between patient homes while delivering one-on-one therapy focused on independence and safety.

Qualifications

• Active Pennsylvania Physical Therapist license or eligibility.
• Prior Home Health experience is required.
• OASIS assessments experience required.
• HCHB EMR experience required.
• Current BLS certification.
• Strong clinical reasoning, time management and communication skills.
• Must be able to travel between patient homes.

Responsibilities

• Provide skilled PT services in patients' homes.
• Complete evaluations and develop individualized treatment plans.
• Perform and document OASIS assessments.
• Use HCHB for clinical documentation and care management.
• Improve strength, balance, mobility, safety, and independence.
• Provide gait and transfer training, therapeutic exercise, and fall prevention.
• Assess home safety and recommend assistive devices.
• Educate patients and caregivers on home exercise programs.
• Collaborate with physicians, nurses, and the Home Health team.
• Complete timely clinical documentation.
