Ardor Health Solutions is seeking a travel Home Health Physical Therapist for a 13-week assignment in Cascade Locks, Oregon. This position features a 40-hour work week, days shift, and travel-based employment within Home Health, offering independence in patient care amid the Columbia River Gorge.

Candidates should hold an Oregon PT license or Compact PT, with preferred Home Health, OASIS, and HCHB experience, plus BLS certification and a valid driver's license.

Qualifications

• Active Oregon Physical Therapist license or Compact PT license.
• Home Health experience preferred.
• OASIS experience preferred.
• HCHB experience preferred.
• Current BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient goals.
• Help patients improve mobility, strength, balance, safety, and functional independence.
• Provide education on home exercise programs, fall prevention, and safe mobility.
• Communicate patient progress and needs with physicians, caregivers, and the interdisciplinary team.
• Complete accurate and timely clinical documentation.
