Cross Country Allied is seeking a Physical Therapist (PT) with a Home Health focus for travel assignments in the White River Junction, VT region and surrounding areas in NH/VT. This full‑time travel position involves evaluating, developing, and implementing PT plans of care, coordinating with an interdisciplinary team, and educating patients and families.

The role requires a BS in Physical Therapy, current VT/NH licensure (or compact license), and at least one year of home health PT experience.

Qualifications

• BS in Physical Therapy required.
• Licensed Physical Therapist in VT and NH, or unencumbered compact license.
• Minimum of 1 year of home health PT experience.

Responsibilities

• Evaluate and develop individualized PT treatment plans for home health patients.
• Implement and monitor treatment plans; adjust as needed in collaboration with the team.
• Educate patients and families on therapies, goals, and procedures.
• Screen, evaluate, re‑evaluate, and treat patients in home health settings.
• Document evaluations and treatment goals; maintain accurate records.
