Job summary

Ardor Health Solutions is seeking a Travel Physical Therapist for a 13-week home health assignment in St. Albans, Vermont. You will provide skilled PT services in patients' homes, helping them regain mobility and independence while traveling between cases.

The role offers a Monday–Friday schedule, days, with weekly pay and comprehensive benefits. The ideal candidate has a Vermont PT license, home health experience, and enjoys working independently in a mobile caseload.

Qualifications

• Active Vermont Physical Therapist license or PT Compact privilege.
• Graduate of an accredited physical therapy program.
• Home health experience preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and manage a mobile caseload.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive physical therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercise, gait training and balance activities.
• Assess home safety and recommend appropriate adaptive equipment.
• Help patients improve functional mobility and independence.
• Educate patients and caregivers on home exercise programs.
• Coordinate care with physicians, nursing and other healthcare professionals.
• Monitor patient progress and adjust treatment plans.
• Maintain accurate clinical documentation.
• Travel between patient homes within the assigned service area.
