Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Belle Fourche, South Dakota. This 13-week contract focuses on SNF/LTC settings with 40 hours per week and day shifts.

The role requires graduation from an accredited program and current licensure as a Physical Therapist, with duties including developing care plans, delivering skilled PT services, and collaborating with the care team to achieve patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in the process of licensure.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Join a SNF/LTC setting on a 13-week travel contract.
