Qualifications

• Must have Doctorate in Physical Therapy or Bachelor's in Physical Therapy and NPTE passed.
• Must have graduated from an accredited school.
• Minimum one year's experience.
• Current state license or in process for licensure as PT/Physical Therapist.
• Current CPR certification.

Responsibilities

• Develops a plan of care for each physical therapy patient.
• Provides skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
