Job summary

Apex Home Health, Hospice & Home Care is seeking a dedicated physical therapist to provide services across settings, evaluating, diagnosing, and treating patients with physical impairments while developing personalized treatment plans and collaborating with care teams.

You will use manual therapy and specialized equipment, document interventions, educate patients and caregivers, and participate in discharge planning, with a focus on safe practice and regulatory compliance.

Qualifications

• Valid physical therapy license and BLS certification.
• Strong clinical and communication skills; ability to document effectively.
• Ability to work under pressure and adapt to irregular schedules; PC proficiency.

Responsibilities

• Assess patients and develop rehabilitation programs.
• Operate therapy equipment safely and monitor usage.
• Document interventions and track progress.
• Educate patients and caregivers on self-management and injury prevention.
• Participate in discharge planning and ensure safety and regulatory compliance.
