Job summary

The Judge Group is seeking an experienced RN Hospice Case Manager to provide compassionate, high-quality end-of-life care in the Springfield, MO area. The RN will manage patient care, coordinate with the interdisciplinary team, and support patients and families throughout the hospice journey.

This temporary role runs 3 months with a potential contract-to-hire. Expect a Monday-Friday schedule (8:00 AM–4:30 PM) and travel within a 50-mile radius.

Qualifications

• Active RN license in good standing.
• Current BLS certification.
• Hospice nursing experience preferred.
• Ability to travel within a 50-mile radius of Springfield, MO.

Responsibilities

• Provide skilled nursing care and case management for hospice patients.
• Conduct patient assessments and develop care plans.
• Coordinate care with the interdisciplinary team and participate in IDG meetings (every other Wednesday).
• Maintain accurate patient documentation and ensure compliance with hospice standards.
• Support patients and families with education and end-of-life care needs.
• Participate in on-call rotation as assigned.

Key Responsibilities

• Provide skilled nursing care and case management for hospice patients.

• Coordinate care with the interdisciplinary team and participate in IDG meetings (every other Wednesday).

• Support patients and families with education and end-of-life care needs.

Requirements

• Active Registered Nurse (RN) License.
• Current BLS Certification.
• Hospice nursing experience preferred.
• Ability to travel within a 50-mile radius of Springfield, MO.

Preferred Qualifications

• Experience with HCHB (Homecare Homebase).
• Previous hospice case management experience.

On-Call Requirements

• Weekday On-Call: 4:30 PM-8:00 AM (3 days/week)
• Weekend On-Call: Saturday 12:00 AM-Monday 8:00 AM (2 weekends/month)
