Responsibilities

• Provide patient and caregiver education on disease management and treatment plans.
• Perform venipuncture and other skilled nursing procedures.
• Provide wound care, including experience with wound VACs.
• Administer and manage IV therapy as needed.
• Document patient assessments, interventions, and progress accurately and timely.
• Coordinate care with physicians, caregivers, and other members of the healthcare team.
• Participate in on-call and weekend coverage as required.

Home Health Clinical RN

Location: Clearfield & Centre County, Pennsylvania

Schedule: Monday-Friday, 8:00 AM-4:30 PM

Weekly Hours: 36 hours

On-Call/Weekend Requirement: Yes

Mileage Reimbursement: Yes — mileage reimbursed between patient visits, from the first patient to the last patient

Pay Rate: $50/hr

About The Role

We are seeking a dedicated Home Health Clinical RN to provide high-quality nursing care to patients in their homes throughout Clearfield and Centre County. This position is responsible for conducting OASIS assessments, developing individualized care plans, and providing ongoing return visits to support patients' health and well-being.

Key Responsibilities

• Conduct comprehensive OASIS assessments for home health patients.

• Provide skilled nursing care during scheduled return visits.

Required Qualifications & Experience

• Active Registered Nurse (RN) license in Pennsylvania.
• Previous home health nursing experience.
• Experience with OASIS assessments.
• Experience with HBHC.
• IV therapy experience.
• Wound care and wound VAC experience.
• Experience providing patient disease education and teaching.
• Venipuncture experience.
• Strong clinical assessment, documentation, and communication skills.
