WHAT WE'RE LOOKING FOR

Job Summary
Responsible for providing, under the direction of a physical therapist, therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. Demonstrates accountability for and contributes to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary environment.

Roles and Responsibilities

1. Provides therapeutic interventions as directed by the physical therapist in campus locations, home health and/or outpatient care.
2. Travel to other locations up to and including overnight stays in a designated territory.
3. Evaluates patient response to treatment and provides feedback to the rest of the team.
4. Documents the course of patient care including progress made and continuing need for treatment.
5. Collaborates with the physical therapist to develop new methods of delivering education to patients and family.
6. Assists the physical therapist in providing team members with in-service/cross-training on physical therapy knowledge and skill areas as needed.
7. Contributes to the delivery of cost-effective care.
8. Reports any complaints and/or grievances made by the residents to the Rehab Program Director or Director of Health Services.
9. Reports incidents or suspected incidents of resident abuse to the Rehab Program Director/Director of Health Services and/or the Executive Director immediately.
10. Complies with all resident rights per the company’s Resident Rights Policy.
11. Maintains professional/technical competencies and proficiencies for job responsibilities.
12. Participates in survey preparation activities.
13. Other duties as assigned.

Qualifications
Education: Associate Degree
Experience: 0-1 years

Licenses and Certifications
Current CPR Certification for healthcare providers through CPR training that includes hands-on practice and in-person skills assessment preferred.

Physical Requirements
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
