Job summary

Affinity Home Health Care Inc. is seeking a Travel Physical Therapist to provide skilled rehabilitation services across patient settings, assess needs, and develop and implement effective treatment plans.

You will collaborate with physicians and healthcare teams, document progress, and educate patients and families while adjusting plans to achieve goals. Licensure and NPTE eligibility are required; travel reimbursement is provided.

Qualifications

• Doctorate or Bachelor's degree in Physical Therapy.
• NPTE pass or eligibility must be met.
• Graduation from an accredited school.
• Minimum of one year of experience as a PT.
• Current state license or in application process.
• CPR certification.

Responsibilities

• Conduct patient assessments and set realistic goals.
• Document progress and modify care plans as needed.
• Collaborate with multidisciplinary healthcare teams.
• Develop and implement individualized care plans.
• Educate patients and families on treatment and progress.
