Job summary

Northshore Home Health Care is seeking a dedicated Physical Therapist to assess, plan, and deliver rehabilitation services in a home health setting, developing individualized care plans and educating patients and families.

The role requires a Doctorate or Bachelor's in Physical Therapy, NPTE, current state licensure or active application, CPR certification, and at least one year of experience. Day shift, full-time with comprehensive benefits.

Qualifications

• Doctorate or Bachelor's degree in Physical Therapy.
• Pass the NPTE and hold state licensure or be in the licensure process.
• CPR certification; minimum of one year of experience.

Responsibilities

• Assess, plan, and provide rehabilitation services; develop individualized care plans.
• Deliver skilled physical therapy interventions and educate patients and families.
• Evaluate progress, document outcomes, and ensure documentation compliance.
• Assess referred patients, set goals, modify plans as needed.
