Job summary

Qualifications
- Must hold a Doctorate in Physical Therapy OR a Bachelor's in Physical Therapy with NPTE
- Must have graduated from an accredited school
- Minimum one year's experience
- Current state license or in process for PT licensure
- Current CPR certification
Responsibilities
- Assess referred patients requiring rehabilitation and provide direct patient care
- Establish and administer a treatment program with goals based on patient capacity
- Set realistic goals, document patient condition, and educate patients and families
- Develop a care plan and provide skilled PT services per physician orders
- Collaborate with other disciplines to meet patient goals
